Operating a university vehicle

Employees seeking approval to drive University owned, leased, or rented vehicles for University purposes are required to submit to the following three:

Fair Credit Reporting Act Disclosure Statement
Acknowledgment & Authorization for release of motor vehicle record information
Driver Application Form

Defensive Driving Training
Fuel Key FOB
Insurance

Responsibility for Vehicles
Parking Restriction
Vehicle Purchase/Lease

Vehicle Signage
Vehicle Tags, Titles, Registration, & Delivery

Failure to fully complete or submit any of the forms may delay processing. An employee is not approved to drive unless and until Risk Management confirms their approval.
