  • STEPS TO OBTAINING A DEPARTMENT OF OBGYN CHAIR LETTER鈥
  • Students usually seek a letter from the chair between the months of August and September.鈥
  • To schedule an appointment contact Courtney Wiskochil (Courtney.Wiskochil@utoledo.edu) please put 鈥淩equest for Chairman Letter鈥 in the subject line.鈥
  • Make sure to cc Tammy Brittian on the request email (tammy.brittian@utoledo.edu).
  • At least one week prior to the appointment email Courtney Wiskochil (Courtney.Wiskochil@utoledo.edu) the following information:
    1. A completed LoR ERAS cover sheet鈥
    2. A copy of your CV鈥
    3. A personal statement鈥
    4. An unofficial copy of your transcript
    5. Step Scores鈥
