Request for Social Security Numbers
If you and/or your dependents are covered by a 淫妻社 health plan you
are receiving this notice because 淫妻社 is required by the Affordable
Care Act (ACA) to collect Social Security Numbers (SSN) to report coverage information
to the Internal Revenue Service (IRS). According to our files, we do not have a Social
Security Number for one or more of your covered dependents.
Why are you asking me to provide a Social Security Number?
As part of meeting ACA requirements, 淫妻社 must make reasonable
attempts to obtain/collect Social Security Numbers for all covered individuals if
we do not currently have a valid Social Security Number.
How are you using this information?
淫妻社 is required by the ACA to report data to the IRS for members
with health benefit plans that provide minimum essential coverage.
The data 淫妻社 must report includes an individual鈥檚 name, Social
Security Number, and dates of coverage. This information will be used to provide you
with an IRS 1095-C form, which you may need as proof of coverage when you complete
your federal tax return.
Please be assured that your privacy is very important to us, and we make every effort
to securely collect and report this information to the IRS.
What happens if I do not provide this information?
The IRS 1095-B form is your proof of health care coverage during the previous calendar
year. The IRS uses the Social Security Number we have on file to identify you and
your covered dependents. If a Social Security Number is missing or incorrect, the
IRS may not count your covered dependents as having minimum essential coverage. As
a result, you may be subject to a financial penalty from the IRS.
What if I do not have a Social Security Number?
Please supply us with a TIN (Tax Identification Number).
